Abstract

Purpose: The recurrence rate of surgical problems in Crohn’s disease (CD) in children is high. The aim of this study is to illustrate clinical feat ures and compare outcome of biologics to non-biologic treatment in children with CD who underwent surgery. Methods: The study subjects included children diagnosed with CD who underwent surgery in the Seoul National University Hospital from January 2000 to June 2017. We retrospectively analyzed the medical data of 64 children with CD. We compare the duration from diagnosis to operation in biologics (infliximab, adalimumab), and non-biologic treatment group. Results: Sixty-four cases of children with surgical episode were included. The mean age at diagnosis is 11 years, sex ratio is M:F 41:23 (1.8:1); mean age at primary operation is 14 years; mean time from diagnosis to operation is 36 months. Primary surgery was performed in 15 patients (23.4%) with fistulectomy, 11 patients (17.2%) with Rt. hemicolectomy, 11 patients with perianal abscess drainage (17.2%) and 10 patients with small bowel resection (15.6%). In the patients who had recurred more than 2 times, the average number of operation was 2.9 times in the patients who underwent Ileostomy, and 2.3 times in the fistulectomy patients. Of the 64 patients, 47 (73%) were treated with biologics, of which 19 (40%) were preoperative and the rest were biologics after surgery. In patients who received biologics, the difference in mean time from diagnosis to first operation was 29 months compared with 54 months in patients without biologics, but the difference was not statistically significant (p-value 0.96). There was no significant difference in the interval from diagnosis to the second operation in the biologics group and the non-biologics group. Conclusions: This study found 64 cases of pediatric CD with surgical intervention from a single center during a 17-year period. Clinical characteristics illustrated and relationships with biologics in this study can be helpful in determine treatment guideline. Further research is needed to determine the appropriate timing of surgery and biologics treatment.
